A To Z Group Homes provides full-service assistance to our residents. Before a resident is enrolled in our group home, we conduct an assessment to ensure that our team learns and is aware of the unique needs of our residents. We will then customize a care plan that fits the current health condition and care requirements of the patient.

Integration Home Care
A To Z Group Homes offers supportive services for individuals who wish to maintain their independence while still receiving the help and guidance they need. Through our integration home care program, we promote safety, comfort, and active participation within the community.
Services Include:
- Individual Community Living Supports (ILCS): Designed for individuals who need reminders, prompts, moderate supervision, or physical help to continue living in their own homes.
- 24-Hour Emergency Assistance: Provides immediate response, counseling, and problem-solving support during health or personal emergencies that occur at home.
- Companion Services: Offers non-medical support, social interaction, and supervision for adults 18 years and older.
- Homemaker Services: Includes a range of tasks from light household cleaning to managing daily routines and home organization.
- Individualized Home Supports Without Training: Delivers supervision and assistance to adults or children who need help with community participation, health and safety, home management, or adaptive daily living skills.
- Night Supervision: Ensures overnight monitoring and assistance by staff to maintain comfort and security within the home.
- Respite Care Services: Provides temporary care to give family members or primary caregivers time for rest and relief.
Comprehensive Home Care
Our comprehensive home care services are licensed by the Minnesota Department of Health (MDH), allowing us to deliver a wide range of professional care and support. This program ensures that clients receive personalized health-related services in their homes or other care settings.
Services Include:
- Home Care Nursing: Skilled nursing services provided by registered nurses (RNs) or licensed practical nurses (LPNs).
- Home Health Aide Services: Assistance with activities of daily living such as bathing, dressing, and meal preparation.
- Therapy Services: Physical, occupational, and speech therapy are conducted in the comfort of the client’s home.
- Personal Care Services: Support with personal hygiene, grooming, and overall wellness.
